Gateways - Children
PROGRAM OVERVIEW
The Gateways program provides families who have a child(ren) with a developmental disability information about resources in the community and may also assist them to get connected to various services appropriate to the child(ren) with special needs.
Families must be eligible for FSCD, (Family Supports for Children with Disabilities).
SCOPE believes that children develop within the context of their families as well as their communities. Strengthening both are important goals.
TYPES OF SUPPORT
How we work:
- We evaluate the needs, strengths and resources of the family.
- We assist families with lifelong planning to meet the needs of the child and family.
- We support families to connect with each other and with community services.
- We assist families to develop skills to become effective care managers.
- We offer short-term support during major life transitions or times of stress.
- We work collaboratively with other agencies to identify and begin to address gaps in services within the community.
- We assist families in planning for relief and respite.
- We provide an outreach model of service.
- We advocate for families.
